Move a rogue IP extension into the HQ, does the problem still exist? This will isolate if the issue is with the IP phone setup or the connectivity between the site 2.
You need to do some more investigation before throwing complexity into the network 1. Maybee VLAN & QoS could solve the problem too but i find it difficult to setup and maintain. My solution was to setup 4 ports on each switch with LACP as a link between the 2 switches, after that we had no problems. The setup was 2 x 48 port GBit Switches, on switch1 all of the servers connected + Firewall, users on that switch had no problems, all users on switch2 had voip problem and sometimes the loggin proces taking ages. We had a similar issue where only half of the users reported problems with voip.

Hello, it is time to implement VLAN & QoS or even better upgrade to 2 or even 4 GBit if possible, 100 MBit Link between the buildings is not enough.
It's as much of an opportunity for you as it is a problem. This is an excellent time to redesign the network for other things as well, a focus on resiliency and security also come to mind and you can take your network engineering knowledge to the next level. I'm all for you learning what you need to know as opposed to simply buying the required expertise because once they finish and walk out the door so does the knowledge. You can leverage the network investment you have by making it perform the very best it can (with what you have). As networks grow in an ad-hoc fashion without proper planning they eventually reach a point where such an exercise becomes necessary. If you are not familiar with QoS (and yes the extra complexity that comes with it), go take a course (as opposed to just diving into the deep end) or retain the services of a reputable network engineer to redesign the network properly. I have to disagree with the 'before throwing complexity into the network' it isn't doing what you want it do (delivering voice clearly) and is more than capable of doing so if properly configured.
No matter how much bandwidth you have on your links, if the network doesn't see voice traffic any different than coping a large file from a file server then you are likely to have voice quality issues from time to time. While some people might suggest just throwing more bandwidth at it, VoIP will work well on a very oversubscribed T1 (1.44 Mbps) with QoS properly configured. This sounds like a network that could definitely benefit from QoS. What should I do? Similar complaints are emerging from branch office which is connected back to HQ by Fiber (100Mbps) too but they have only 20 extensions. All switches are connected using SFP slots (Fiber) and NetGear switches are stacked with HQ switch as management. Three NetGear switches are in new building. We use NetGear GS752TPS PoE switches with one Dell Powerconnect 5448. Setup is, we've 60 users (150 devices) in each building and they all are on single subnet 192.168.10.0/23 (one flat network).
